I GIVE permission for Swim Life with Coach Taylor to photograph and/or record my child during swim lessons or related activities. These images or videos may be used for marketing, social media, website, and promotional purposes. No child’s full name or personal information will be shared.

I DO NOT give permission for my child’s image or likeness to be used.

All media is the property of Swim Life with Coach Taylor and may be used without compensation unless permission is revoked in writing.

Assumption of Risk Acknowledgment

I have read and understand that swim lessons involve inherent risks, including but not limited to slips, falls, water-related injuries, and other unforeseen risks. I voluntarily allow my child to participate and accept these risks.

I do NOT acknowledge the risks associated with swim lessons.

Registration Acknowledgement

I understand that while Swim Life with Coach Taylor takes reasonable precautions to provide a safe learning environment, no activity involving water is without risk.
